• Physical assessment and review of medical history to identify appropriate preventative measures.

• Match client and/or family with local or national support groups, financial assistance and other resources as needed.

• Provide education about your diagnosis, health condition, and treatment plan.
• Demystify medical terminology so you can better understand your health situation and set health goals.
• Help define which questions to ask your doctors in order to promote shared decision-making, and make sure you know your rights.
• Coordinate care with your healthcare team, and advocate for high quality and respectful care that centers around you.
• Explore alternative treatment options, including clinical research trials, newer techniques, etc.
• Coordinate health appointments, helping to ensure you are seeing the right types of doctors and making sure providers are in-network with your health plan or accept Medicare/Medicaid.
• Help to organize records and ensure insurance claims are paid correctly per your policy.
• Help you navigate from a hospital admission to home or to skilled nursing facility, or to long-term care. We can help make sure you understand your options, how to finance your care, and ensure you receive the services you need.
• Coordinate with family members, if desired, so they are aware of your health status and how best to support you.
