Do you have a hospital admission or outpatient procedure planned and need help once you are discharged home? Breakaway Health Solutions can provide guidance by a Registered Nurse to help you transition home.
Even though your doctor may order home health services when medically necessary, you should know that insurance will only cover a limited number of hours per week. And, in order for insurance to cover the services of an aide for personal needs such as bathing and dressing, meal preparation and light housekeeping (activities of daily living), you must also require the care of a licensed professional such as an RN, a respiratory therapist, physical therapist or speech pathologist. Otherwise, you must rely on your own personal support system or pay out of pocket for such care.
Breakaway Health Solutions Transitional Care is designed to fill the gaps between your personal support system and services that are covered by your insurance plan.
When scheduled in advance, Breakaway Health Services can coordinate with your provider team prior to your procedure to ensure your discharge plan is appropriate for you. Whether you will need medications, home health services, medical equipment, and/or medical supplies, we will work to make sure everything is lined up and approved in advance of your procedure, and that the providers are in-network with your insurance plan. We will coordinate your care to make sure you have the appropriate follow-up appointments and transportation, and that you understand your medication regimen and your discharge instructions. If desired, we can visit you during your admission to monitor your care and advocate for you. Once you are home, a Registered Nurse can help you manage drains or other medical devices, assist with IV, oral or injectable medications, assess dressings and provide dressing changes as appropriate, monitor for adverse reactions, assist with activities of daily living, and more. We’ll even help take care of your pet while you are out of commission!

Transitioning home from a Skilled Nursing Facility (aka SNF or "rehab") can be stressful. Discharge can often be rushed, making it difficult to set up home services and family support in a timely manner. And, while most people are glad to be home, a changed health condition can make it more difficult to take care of yourself at home. Breakaway Health Services can make that first day home easier for you. We can coordinate with the SNF prior to your discharge to make sure you have the equipment you will need to be safe at home, that your medications have been ordered, and that home services are ordered as appropriate. We can intervene with your insurance company (with your consent) if there is trouble getting equipment or services approved. We can provide guidance so you understand your rights and how to appeal a decision if need be. And we will be there when you arrive home to make sure you understand your medication regimen and assist you to be safe as you get settled.
